Reservations for the Practitioners Day Forum are limited in number, so early registration is recommended. Registration must be completed in advance. There will be no on-site registration for this event. Participants may register online here or may click here for a pdf form.

Practitioners Day Forum

Full Registration

Full registration includes entrance to the Icebreaker & Early-Bird Reception, all Program Sessions, the Annual Meeting Luncheon, Annual Meeting Reception and Annual Meeting Banquet. The registration fee is $150.

Daytime Registration Package

A daytime registration package is also available. It includes entrance to the Icebreaker & Early-Bird Reception, all Program Sessions, the Annual Meeting Luncheon and Annual Meeting Reception. The registration fee is $100.

Program/Event Cancellations

Cancellations must be made in writing to MCEER by February 9 , 2005. No refunds will be given for cancellations after February 9 , 2005.
